1. THE PHENOMENON
Leo Tolstoy wrote that happy families ar all alike; any unhappy family is unhappy in its own way (1877). I suspect that helplessness information systems, like Tolstoys families, are each failing in its own way inside an implied cultural framework that focuses on succeeding. Two important elements in systems failing - relative to the research contemplated here - are first, that the failing is generally non acknowledged until a manager or leader declares it to have failed and second, that a major problem with poring over information system failure is that most organizations are not likely to publicly disclose information about failures (Rocheleau, 1997). This does not imply that the process of failing goes unnoticed before it is acknowledged.

For the purposes of this exploration, information failure will be defined within the context of information system failure and will be limited to information systems that are failing to function as intended.
